AESTHETIC PLASTIC SURGERY







Buttock augmentation or gluteoplasty requires careful evaluation since the anthropometry of this area must be evaluated because the morphology is individually given by the insertion of the gluteus maximus muscle and the bone disposition of the hip.
There are two alternatives to increase the volume of the hips; the first is fat grafting, for which prior liposuction is required in the same surgical act.
The second alternative is the placement of gluteal prostheses.
It is possible to combine both techniques, placing the implants and complementing the result with fat grafting to give the hip a rounded shape and harmonize this area with the waist.
I do not recommend infiltrations of substances such as biopolymers, hyaluronic acid, fillers, liquid silicone, paraffin, or other types of substances since many complications and rejections have been described with these techniques, with pain, inflammation and malformations that are difficult to treat and correct.
HOW IS THE SURGERY PERFORMED?

Buttock augmentation or Gluteoplasty can be performed basically by two techniques:
Buttock Implants:
They are inserted through the incision in the Intergluteal fold, practically invisible to the eye; the implants are made of highly cohesive medical-grade silicone and specifically designed to give shape and contour to the gluteal region.
They are placed inside the gluteal muscle and significantly improve its appearance, with sufficient muscle, subcutaneous cell tissue and skin coverage, for which the result is natural. Different techniques with incisions or skin scars in other locations on the hip have been described. Likewise, different planes where the pocket is located are described, such as the subcutaneous, subfascial, under the fascia, or retro muscular. Techniques that I usually do not recommend these due to the possibility of obtaining a less natural result or technically more complex to perform.
There are several forms of implants: round, anatomical or Quartz, each one with its indications, advantages and disadvantages that will be discussed in the consultation and the best option will be defined according to the patient’s preferences and the physical examination.
Gluteal Lipoinjection – BBL Brazilian Butt Lift:
It is the most widespread technique, also known as BBL (Brazilian Butt Lift), which consists of implanting the patient’s fat extracted from the abdominal region, back or legs and injecting it into specific sites at the gluteal level to achieve harmonization and improve the contour. It must be taken into account that fat usually presents reabsorption of 50%, a percentage that may be higher or lower depending on the characteristics of each patient.
Depending on the case, a patient may be a candidate for one or another procedure or a combination of them; basically, it depends on the anatomy, the expectations, and the amount of fat available.

- Pain: The pain is mild and controlled with the medications ordered after the surgery. The analgesic pain pump is commonly used in this surgery for postoperative pain control, installed by the nursing team with the support of the anesthesiologist and outpatient follow-up for 2 to 3 days. Sleeping facing down or lateral is recommended for the first week after surgery.
- Inflammation: During the first days, you will notice inflammation at the level of the buttocks, and some bruises may appear, which will gradually decrease.
- Return to activities: After seven days, the patient can gradually return to daily activities.
- Exercise: Wait 3 to 4 weeks to exercise; avoid entering a swimming pool until the scars are entirely closed, which can take 2 to 4 weeks after surgery.
- Position: During the postoperative period and after adequate healing, there are no limitations to sitting because the support site where the body’s weight is exerted is the ischial region, and the prostheses and the fat graft are not located there.
- Buttock shape and natural result: It is common to hear from patients that hip augmentation gives an unnatural result, particularly with implants. The results are usually satisfactory because the implants are positioned between the gluteal muscle, improving harmony and volume but not visible under the skin.
